This program provides care management services to people with Multiple Sclerosis through a contractual agreement with the National Multiple Sclerosis Society – Greater New England Chapter. Case managers meet with clients in their homes and work with them to create an individualized care plan which includes identifying available local services and resources and assisting clients to navigate enrollment in those services.
